Given the high demand, increased expense and the emphasis on patient-centered care, health care providers are being challenged to create new options for families who want to think differently and manage every aspect of their health care experience. Thoughtfully managing care transitions that decrease readmission and improve outcomes, developing innovative cross industry relationships while integrating technology have become a new health care imperative.
